The following options are available for configuration for checklist Question Design records, with some options available where indicated for configuration for checklist Available Answer Design records. Depending on your configuration, you may need to update page layouts to include available options:

  • Question Type: Specifies whether the question is a Date, Multiple Choice, Number, or Text Question.
  • Order: Specifies the order the questions will appear in each section. This option is also required for configuration for checklist Available Answer Design records.
  • Question Text: Provides the question to which users are asked to respond.
  • Question Guidance Text: You can optionally provide up to 300 characters of text per question to guide checklist respondents in how to answer a question. When configured for a question, users can hover over the question to see the question mark icon (?) in the top right corner of the question. Users can then click the question mark icon (?) to display the guidance text. You can configure this option for Question Design records (including Library Question records). To use this option, you may need to update the page layout for Question Design records. Note that if your configuration includes custom object types for Question Design records, you need to assign the Question Text field (question_help_text__sys) to the appropriate custom object types.
  • Checklist Answer Type (Multiple Choice Questions only): Specifies whether the answer type for the multiple choice question is Radio, Drop Down List, or Checkbox.
  • Required Response: Specifies whether responding to the question is mandatory or optional for the user.
  • Comment Allowed: Specifies whether users are allowed to add comments in a response.
  • Comment Required: Specifies whether users must provide a comment in a response. Comments Allowed must be set to Yes for this option to be set to Yes.
  • Attachment Allowed (Attachment): Specifies whether users are allowed to attach files in a response.
  • Attachment Required (Attachment): Specifies whether users must attach files in a response. Attachment Allowed (Attachment) must be set to Yes for this option to be set to Yes.
  • Attachment Allowed (Document): Specifies whether users are allowed to attach a document in a response but only at the Question level for Number, Text, Date, and Multiple Choice Questions. As of the 23R2 release, this field is no longer active.
  • Attachment Required (Document): Specifies whether users must attach a document in a response. Attachment Allowed (Document) must be set to Yes for this option to be set to Yes. As of the 23R2 release, this field is no longer active.
  • Documents Allowed: Specifies whether users can add related version-specific documents in their response at the Question level for Number, Text, Date, and Multiple Choice Questions or at the Answer level for Multiple Choice Questions. For multiple choice questions, this option is also available for configuration for checklist Available Answer Design records. Note that a Multiple Choice Question cannot have Documents Allowed set to Yes at both the Question and Answer levels. Users can add a maximum of 10 documents.
  • Documents Required: When Documents Allowed is set to Yes, this option specifies whether users must select a Single Document, Multiple Documents, or whether selecting documents is Not Required (optional) as part of their response. For multiple choice questions, this option is also available for configuration for checklist Available Answer Design records. For multiple choice questions, ensure you configure this option only for the Question Design record or the Available Answer Design record but not for both. Users can add a maximum of 10 documents.
  • Minimum Answer Value (Number Questions only): Specifies the lower range limit for valid responses as a number between -100,000,000 to 100,000,000, including up to 9 decimal places.
  • Maximum Answer Value (Number Questions only): Specifies the upper range limit for valid responses as a number between -100,000,000 to 100,000,000, including up to 9 decimal places. The Maximum Answer Value must be greater than the Minimum Answer Value for number questions.
  • Decimal Places (Number Questions only): Specifies the number of decimal places allowed in response to a number question as an integer from 0 to 9. The default if you do not specify a value for this field is 0 and checklist respondents must enter an integer (no decimal places) in number question responses.
  • Maximum Selectable Answers (Multiple Choice Questions only): Specifies the maximum number of answers which users can select in a response. For Checklist Answer Type records of Radio and Drop Down List type, this value is 1. Ensure you create the appropriate number of corresponding Available Answer Design records in order to change this value.
  • Score (Multiple Choice Questions only): Score is calculated against Weight% option that is set up for each section in the Question level and by using the points value of each answered question in the Answer level.
  • Reference Document(s): You can optionally add up to five (5) documents per question with more related information. When configured, a reference document appears as a link that the respondent can click to view the document in a pop-up window.

Checklist Limits

The following maximums apply to checklists:

  • Sections per checklist: 20
  • Questions per section: 50
  • Answers per question: 20
